    ANNUAL PARENT NOTIFICATION
    Please review the county's Annual Parent Notice along with our school district's important documents. Then, please print, complete, and return the Mandatory Signature Page and Health Inventory attached at the bottom of this webpage. You may hand-deliver or mail these signed documents to your school. If you prefer a printed packet, it is available at your school office.
